Bridges Self-Management Limited runs individualised training programmes for health and social care professionals to enable them to engage, empower and support patients with long-term conditions to self-manage their rehabilitation and enhance their lives during and post-rehabilitation.
It is a programme built on significant research evidence and is based on the experience of stroke survivors, carers and practitioners. It has made a significant difference to those that have used it and we are seeking to grow the programme whilst ensuring we retain the interests of the patients that the programme is designed to support.
Bridges was set-up following the successful “spin-out” of the Bridges Stroke Self-Management programme from the joint faculty for health and social care sciences at Kingston and St George’s Universities of London. Bridges is a social enterprise with ambitions to scale up and support many more stroke survivors as well as branching out and supporting patients with other long-term conditions to self-manage their rehabilitation.
