Discuss, Decide and Do Your Healthcare Directive
It’s about conversations. It’s about decisions. It’s about communicating what’s important to you. Join us for National Healthcare Decisions Day. Learn more about Advance Care Planning and what it means to you and your loved ones. This event is designed to meet you wherever you are in the process!
DISCUSS | Regardless your health or age, we all need to make decisions about our end-of-life wishes. A Healthcare Directive is a legal document that communicates your wishes regarding medical care when you are no longer able to. Offered four different times throughout the day, attend an informational sessions that will provide an overview of Advance Care Planning with a panel of experts available to answer your questions.
Four sessions times. Pick the time that best fits your schedule. -12:30 p.m.-2:30 p.m.-4:30 p.m.-6:30 pm.
DECIDE | Following each session there will be opportunities for small group discussions to assist you in gaining clarity on what is important to you as well as opportunities for 1:1 support to assist you with questions or roadblocks as you fill out your Healthcare Directive.
DO | Notaries will be on site to legalize finished documents and provide you with details on next steps.
What to Bring
Nothing—just an open mind. All necessary forms and information will be provided. Refreshments and snacks will also be provided.
All adults 18+ are welcome to attend. Friends, family and community members are encouraged to come along and start the process, too. You do not need to be a Gilda’s Club member to participate.
Supervised playtime for children is available upon request. Parents or guardians must sign up each child at least 72 hours in advance to ensure coverage during your selected session. Contact Anne Josten at 952-767-7623 or firstname.lastname@example.org to sign up your child.
