Ace Home Healthcare clinicians are trained in geriatric patients and provide each person with a complete assessment and an individualized plan of care that incorporates each patient’s goals, in cooperation with the medical doctor. Allowing each patient to participate in their care and achieve the maximum level of independence. Utilizing an intra-disciplinary group approach of nurses, physical therapists, occupational therapists, social workers and certified nurse assistants working together to achieve the patient’s success.
