Misja The Tufts Health Plan Foundation’s mission is to improve the health and wellness of the diverse communities we serve.
Opis Building age-friendly communities is essential if our society is to remain relevant and responsive to this growing population. Viewing older adults as an asset is critical as they age in place, work to live healthier lives, and contribute their experience and knowledge to our communities. We invest in communities that promote healthy living across the life span with an emphasis on older adults. We move communities, particularly the most vulnerable, toward accomplishing the World Health Organization’s standards for Age-friendly cities.

Healthy Living, Healthy Aging

Healthy aging does not begin at 65 – it is a lifelong process shaped by individual and community-level factors, social determinants as well as nutrition and physical activity. The Foundation will use as a lens for its investments, the World Health Organization (WHO) framework of “active aging” through age-friendly cities. Age-friendly cities can encourage active aging by optimizing opportunities for access to health care and healthy choices, participation in community, and security to enhance quality of life as people age. The Foundation will fund programs that support individual behavior change through community-level systems and policy change.

Collaboration is Key

Systems and policy change require intentional coordination and collaboration among key stakeholders within networks in and outside of the field; community-based organizations, municipalities, health care providers, social service agencies, businesses and consumers – everyone within a community setting and importantly, users of programs and services. To effectively build healthy communities and age friendly cities, organizations must be inclusive in their approach to their work, and work collaboratively maximizing resources to achieve better outcomes.
